FOR IN-PERSON PRIVATE INSTRUCTION
All students, as well as FSL SeniorNet volunteers must be vaccinated and boosted against COVID 19. Please bring documentation of such to the first session. Until further notice, masking and social distancing will also be enforced. The COVID situation is very fluid and restrictions are subject to change.
Use this form to register and pay for your first Private Instruction Lesson and/or up to five (5) follow-up lessons that you have previously arranged with a SeniorNet instructor.
The fee for the first hour is $25 payable three business days in advance. Each subsequent hour is $20. Handouts may be included.
